Release Form for requesting your records for Anne Marie from a previous provider. Complete the form with 1) A previous provider or entire clinic name and location, 2) Your information and signature, 3) Initial all 5 blanks, if possible, for certain types of information to release. Please ask if you need guidance about the type of information to request. Anne Marie will commonly request chart notes and laboratory results for the previous two years. The form may be faxed to our office for tracking or to the previous provider.
